FD3 - Employment Status Referral Form Q3

Is this referral related to services provided by an institution, charity, landlord, catering or accommodation provider, or a company with more than 10 employees?

We do not expect you to know exactly how many employees the company has. The idea is to exclude known large companies from submitting an FD3.

• No - continue to complete the FD3 form

• Yes - Do not continue with this form.
  In order to get tax clearance, please send an email to the tax team: FD3@admin.cam.ac.uk providing the below details:

• Supplier name and supplier number (if already on CUFS)
• Company House reference number (if applicable)
• Supplier’s website (if any)

    And attach a copy of the quote/document supporting agreed terms and condition.
